When to Refer to Wound Care Specialist

Stalled or Worsening Wounds

Wounds showing no improvement after 4 weeks of standard care, wounds enlarging despite appropriate treatment, or wounds developing new areas of necrosis or breakdown. These require specialist evaluation to identify underlying barriers to healing and implement advanced interventions.

Infected or Suspicious Wounds

Signs of infection (purulent drainage, increasing pain, erythema, odor), suspected osteomyelitis (exposed bone, probe-to-bone positive), or wounds not responding to antibiotic therapy. Specialists can perform cultures, aggressive debridement, and coordinate with physicians for appropriate antimicrobial management.

Wounds Requiring Debridement

Significant necrotic tissue, eschar, or slough that requires sharp/surgical debridement beyond the scope of home health nursing practice. Our specialists perform bedside debridement in the patient's home with proper anesthesia and equipment.

Candidates for Advanced Therapies

Wounds that may benefit from negative pressure wound therapy (NPWT), cellular/tissue-based products (amniotic grafts, bioengineered skin), or other advanced modalities requiring specialist evaluation, ordering, and application. We handle all Medicare authorization and documentation.

Home Health Nurse Identifies Need

During routine visit, nurse recognizes wound requires specialist intervention. Nurse contacts Healix360 via phone, fax, or secure portal with patient information and wound description.

Rapid Coordination

We contact patient's physician for orders (or coordinate with home health to obtain orders), verify insurance coverage, and schedule visit within 24-48 hours. We communicate visit time to home health team.

Specialist Home Visit

Our specialist visits patient at home, performs comprehensive assessment and treatment (debridement, advanced dressing application, etc.). Home health nurse can coordinate to be present if desired for continuity.

Ongoing Collaboration

We send detailed visit notes to home health within 24 hours with treatment plan, recommendations for dressing changes between specialist visits, and schedule for follow-up. Home health team continues general care per updated plan.

Regular Communication

Open lines of communication maintained. Home health nurses can reach our team anytime with questions or concerns. We coordinate care plan changes with home health to ensure seamless patient experience.
